What's a middle? If you bet Over X.5 at one book and Under Y.5 at another book where X < Y, you "middle" the game when the final total lands in (X, Y). Both bets win. The wider the window, the more integer totals can hit the middle. Combined hold tells you what the worst-case loss looks like (you lose one of the two bets every time the total falls outside the window).
Example: bet Over 7.5 at Book A and Under 8.5 at Book B. If the game lands on exactly 8 total runs, both bets win. If the game lands on 7 or fewer, only the Under wins. If it lands on 9 or more, only the Over wins.
